PCN Clinical Pharmacist

As a Primary Care Network (PCN) Clinical Pharmacist, I work across local GP practices to support patients in getting the best outcomes from their medicines. I provide expert advice on medication, carry out structured medication reviews, help manage long-term conditions, and work with patients to ensure their treatment is safe, effective, and tailored to their individual needs.

I work closely with GPs, nurses, and other healthcare professionals to optimise prescribing, reduce medicine-related risks, and support patients with any questions or concerns about their medication. I am committed to delivering evidence-based, patient-centred care and helping people understand and manage their medicines with confidence.
